Job Requirements of Home Health Care Coordinator:
- Complete home nursing care visits
- Received clinical to home care cases
- Participate in coordination of home care services
- Ensure appropriate utilization of home care services
- Ensuring home care is delivered post discharge
- Provide verbal authorization to home health care agencies
- Receive all external faxes for home health care services and forward to the home care nurse for review
- Take referrals, and schedule home care services
- Provide home health aide visits
- Support the clinical care team and patient care
- Follow up on home health aide/personal care attendant assignments and patient/client services
- Supervise home health aides and licensed practical nurse in care
- Ensure continuity of care for patients transitioning from a facility to home care environment
- Promote effective assessment for and utilization of home care services
- Create a positive home environment
- Assure that assessments, reassessments, and home care services
- Promote effective assessment for and utilization of home care services
- Instruct the patient and members of the family in home care
- Coordinate home health aide and personal care attendant schedules on a daily basis
- Conducting home visits to military families to evaluate staff who are providing in-home respite care

Home Health Care Coordinator
APR is hiring for a Home Health Care Coordinator to start with our client ASAP!
This role is full time Monday to Friday 830am to 5pm and 4pm on Fridays
Pay range is $17 - $19 phr based on experience with quarterly bonuses
Experience in Home Health Care is a **MUST**
The role itself is as follows
Responsibilities:
- Effectively manage the customer care process and serves as point of contact for existing clients
- Consistently answer office phones professionally and participate in “after hours on-call” rotation (additional compensation)
- Ensure all clients are properly staffed
- Engage and frequently communicate with caregivers
- Assist in the management and coordination of skilled nursing referrals
- Administrative tasks to assist with human resource and recruiting activities, as needed
Additional Skills:

Education:
- Minimum High School Diploma or equivalent, prefer some college
Experience:
· Customer service and client satisfaction
· One (1) year experience in an office environment, in a healthcare setting preferred
· Proficient in MS Office (Outlook, Word, Excel, and PowerPoint); internet
· Excellent organization, planning, and project management skills
